I think we all have an inclination—at least from time to time—to judge others. No matter how experienced, compassionate, and professional we are as physicians, we’re still human. I can admit that I have indeed struggled with patients like the 65-year-old morbidly obese male who complains at every visit about how hard it is to control his blood sugar levels with the medications prescribed. I want to convince him so badly about diet and exercise, but I have tried, and often, these patients don’t want to hear it. Pharmacological or surgical interventions are the preferred route for many.
Empathy as a Path to Healing For various reasons, some patients may challenge your medical guidance and advice. While these encounters can be frustrating, cultivating awareness is the first step toward generating an effective, non-judgmental response. Awareness begins with recognizing and acknowledging your natural emotional response, such as feeling angry when your skill and experience appear to be challenged. Non-judgmental response asks you to hold back and rise above the immediate impulse to become defensive or argue. The next step is the most challenging: showing empathy for the patients who challenge you most.
Canadians are feeling significantly less empathetic than they did before the COVID-19 pandemic began more than two years ago, a new survey by the Canadian Mental Health Association suggests.
Julia Ramsay of the CMHA's P.E.I. division says while that's understandable given the fatigue many people are feeling, the association has made empathy the focus of the 71st annual Mental Health Week May 2-8.
"Empathy is all about connecting, understanding a person's perspective, their feelings, being respectful, practicing kindness," she said, "seeing that decline has really shifted our focus to want to, you know, help people or aid people toward being more empathetic toward others."
The survey, conducted by researchers at University of British Columbia, suggests only 13 per cent of Canadians feel empathetic — compared with 23 per cent at onset of pandemic.

Empathy is a reigning topic in healthcare because of its relationship to the aims of medical practice: improved health outcomes adherence to treatment recommendations, improved quality of care and improved patient and provider experience. At the same time, empathy reduces clinician burnout by providing meaningful connection with patients and alignment with the purpose of the medical profession: to reduce suffering and enhance well-being while also reducing the risk of medical errors and malpractice claims.
While many believe some people are empathic by nature, the capacity to act empathically is mutable and can be depleted in challenging times. Studies have shown that empathy decreases during medical training, and the science of empathy shows that empathy can be downregulated5 and also upregulated by neuroscience-based interventions. Certainly, the last nearly 2 years of practicing in a pandemic has beaten empathy out of many of us, due to the difficult working conditions, persistently high workload and personal anxieties. However, particularly in times like these it is important to pause and find ways to teach—or reteach it.

It is not just patients dying within the walls of our hospitals. Empathy is also dying one by one in the hearts of even our most empathetic health care workers and being replaced by apathy. And while all of us would love to get that empathy back, we need help from our communities. Because we have nothing left to give, there will soon be no one left to give it. And we should all care about that.

Empathy training has been recommended for all staff at Caboolture Hospital after a probe found there was poor leadership and a lack of enthusiasm for safety and quality.
350 subjects, ≥12 years of age received either a standard or enhanced physician visit as part of a randomized controlled trial. The patient-scored Consultation and Relational Empathy (CARE) questionnaire assessed practitioner-patient interaction, especially empathy. Cold severity and duration were assessed from twice daily symptom reports. Nasal wash was performed to measure the immune cytokine IL-8.
Results
84 individuals reported perfect (score of 50) CARE scores. They tended to be older with less education, but reported similar health status, quality of life, and levels of optimism. In those with perfect CARE scores, cold duration was shorter (mean 7.10 days vs. 8.01 days, p=0.032), and there was a trend towards reduced severity (mean AUC 240.40 vs. 284.49, p=0.118).
After accounting for possible confounding variables, cold severity and duration were significantly lower in those reporting perfect CARE scores (p=0.037 and p=0.017 respectively). In these models, a perfect score also correlates with a larger increase in IL-8 levels (p=0.015).
Conclusions
Clinician empathy, as perceived by patients with the common cold, significantly predicts subsequent duration and severity of illness and is associated with immune system changes.
Before we move forward to elaborate on the role of empathy in improving patient care, one must be clear about the definition of empathy. What is empathy? And why it is important in improving patient care.

The therapeutic effectiveness of the provider-client connection is enhanced by good sympathetic communication. Appropriate use of empathy as a communication strategy accelerates health-related interviews, improves data collection efficiency, and acknowledges the patient or client.

Can empathy help us heal health care? Health care is supposed to make us healthier -- but patients and doctors in the US both suffer under the current system. Adrienne Boissy, Chief Experience Officer at the Cleveland Clinic, says the root cause is a lack of empathy. Burnout is a growing problem among medical staff worldwide and empathy has been described as an essential competence to attenuate burnout. Previous studies found job satisfaction and job commitment were affected by the empathy and associated with burnout. This study explores the effect and mechanism of empathy on burnout on medical staff and investigates the mediating role of job satisfaction and job commitment in the relationship between empathy and burnout among medical staff.

Empathic erosion and burnout represent crises within medicine. Psychological training has been used to promote empathy and personal resilience, yet some training useful within adjacent fields remain unexplored, e.g., Stoic training. Given recent research within psychology suggesting that Stoic training increases emotional wellbeing, exploring this type of training within health professions education is important. We therefore asked: What impact would a Stoicism informed online training package have on third year medical students’ resilience and empathy?
Health-care providers can manage this crisis by practicing a specific type of empathy that one of us (Jodi Halpern) calls “empathic curiosity.” It involves trying to understand another person’s world from the inside out. In health care, this means inquiring about the patient’s unique experiences by asking questions such as “tell me more” and “tell me what I’m missing.” Exercising this type of empathy creates space for the patient and doctor to think together and discover novel ways to improve care.

Teaching students empathy and communication
In the 1990s, medical educators realised students’ training was too focused on biomedical sciences and did not take into account the experience of patients and their families. Most medical schools now invest considerable effort to make sure future doctors are well equipped to support their patients and be empathetic practitioners.

Leadership has faced newer challenges as we grapple with the new normal. Practicing empathy as a leader has never been more critical. Empathetic leadership is important because empathy fosters trust, resilience, healing, growth, creativity, and nourishing connections. Empathy reduces stress, transforms conflict, and supports a sustainable collaborative environment. While it is important to develop empathy for others, it is equally important to provide self-compassion for yourself.
In this session, we will explore how a leader can develop empathy for others through self-compassion. Empathy without self-compassion can leave a person drained of energy as a result of feeling what others feel. Our goal is to provide you a framework for self-compassion and self-care so that you can become an empathetic leader who understands others' situations and can communicate this understanding to others and reciprocate as per their needs, all while ensuring their own pitcher is not empty.
A 2011 survey of 800 recently hospitalized patients found that only 53 percent of them felt that their physicians were empathic and caring. And it’s not just in their heads: In one study where doctor-patient encounters were videotaped, researchers found that doctors often overlooked or dismissed signs of distress communicated by patients, providing empathic responses only 22 percent of the time. Other studies have found similar results.
These shortcomings have long dismayed patients like myself—but recently they seem to be troubling leaders in the medical profession as well.
Studies reveal that more than half of all practicing physicians demonstrate signs of burnout. Contemporary physicians face tremendous pressures due to a confluence of factors, including balancing heavy patient loads within constrained schedules, the increasing complexity of patient health problems, and increasingly burdensome COVID-related documentation requirements.
These circumstances—and more—challenge physician empathy, and even to some extent dampen it even further. Multiple research studies document a decline in empathy that appears to begin in the third year of medical school and persists during residency. The pandemic has exacerbated this deterioration. In the past, empathy rebounded after the rigors of training were over, but today, empathy needs to be refreshed to help both patients and providers.
